Dynamic Forms Dashboard: Students looking to track the completion of their forms can log onto their Dynamic Dashboard:
FORMS FOR REGISTRATION
: For use when you wish to withdraw from a course after the last day to Drop but before or on the last day to withdraw without penalty. 聽(Please review the academic calendar for the specific dates)
: 聽This form must be completed, signed by all required parties, and submitted to the Office of the Registrar no later than the last day to Add courses.
: This form is used to apply for permission to add an internship course, not a directed study course. This form must be submitted before the last day to add course.
: 聽For use by matriculated, current students only seeking permission to take courses at other institutions and apply that credit to their 麻豆传媒覆灭记 program of study.
: This form is used to request to audit a class.
FORMS FOR CHANGES TO STUDENT INFORMATION/STATUS
: Students are required to complete and submit this form during their Sophomore year. 聽This form may also be used to indicate a new major, double major, minor, multiple minors.
: This form may be used to request a change to your advisor.
: This form is completed if you do not plan to return to 麻豆传媒覆灭记and/or intend to leave the university during the course of the semester. 聽You may also fill out this form if you intend to leave the University during the ADD/DROP period of the semester.
: This form is required for any undergraduate student who wants to take any THEO class coded 500 level and above.
